If you have already scheduled your first appointment with Healing Pathways Hypnosis Center, please find the appropriate new client forms below.
We ask you to:
1) Print out your forms
2) Fill them out
3) Bring them with you to your first session
This will allow you to take full advantage of your appointment time. If you would prefer to fill out your forms during your first session, please contact our office at (512) 699-2000.

Healing Pathways Hypnosis Center requires 24 hours notice for any canceled appointment. If the client fails to inform Healing Pathways Hypnosis Center staff of their absence with prior 24 hour notice of their scheduled appointment, the client will be responsible for 50.00/hr. due to their missed appointment. We are sincere and dedicated in helping you obtain your life goals. Thank you for your cooperation.
